Insurance Information
UNDERSTANDING YOUR ORTHODONTIC BENEFITS
If you have orthodontic coverage through your dental insurance plan, our team will provide the documentation needed to help you submit your claims and determine your available reimbursement. Insurance is an agreement between you and your insurance provider, and patients are directly responsible for their treatment fees.
Canadian Association of Orthodontists insurance forms provided.
Documentation prepared for insurance claims.
Reimbursement is paid directly by your insurance provider.
Our team is happy to answer insurance-related questions.
Submitting Insurance Claims
After payment for diagnostic records and treatment planning, you'll receive a receipt and a Certified Specialist in Orthodontics Standard Information Form. Attach these documents to your Dental Claim Form and submit them directly to your insurance provider. During active treatment, simply submit the receipts we provide with your Dental Claim Form for reimbursement.
Navigating Your Insurance
GUIDANCE THROUGHOUT THE CLAIMS PROCESS
Whether you're requesting a pre-determination before treatment or submitting claims throughout your orthodontic journey, we're here to help you understand the process and provide the necessary documentation.
Insurance Pre-Determinations
If your insurance provider requires a pre-determination before treatment begins, complete your Dental Claim Form along with the Patient Identification section of the Certified Specialist in Orthodontics Standard Information Form provided by our office. Submit both documents to your insurance provider, who will notify you directly of your available coverage. If you haven't received a response within two to three weeks, we recommend contacting your insurance provider.
During Active Treatment
Throughout your orthodontic treatment, we'll provide receipts for your payments. Attach each receipt to your Dental Claim Form and submit it directly to your insurance provider for reimbursement. In most cases, additional orthodontic forms are not required after treatment has begun.
